ELEVATOR RESERVATION FORM

KEY FOB, GATE REMOTE, BIKE KEY FORM
If you need a new Key Fob, Gate Remote, or Bike Key; please fill out the top portion of this form.
Return your completed form to the office with a $50 deposit (per fob and/or gate remote) in the form of a check, money order, or cashier's check made out to Century Park Plaza. Please allow up to 24hr turn around time for the issuance of your new key fob/gate remote/bike key.
Please note that if you are NOT an owner or property manager; to please have your owner/property manager fill out this key fob authorization form. Please turn in the authorization form, along with the new key fob/gate remote/bike key request form, and deposit to the office.

LOCK BOX STORAGE RENTAL

Lock Box Storage Rental is available for $50.00 per month. Please fill out and return to the office with a check made out to Century Park Plaza for the total amount of months you'd like your rental for (ex. $100.00 would be for two months of rent).
Please be sure to properly label your lock box with your Name and contact information. Please call the Office if you have any questions or concerns.
RESIDENT COMPLAINT FORM
Click HERE. Do you have a complaint that you'd like to put in writing? Please fill out a Resident Complaint Form with as much detail as possible. Please include dates and times. If you need more space; feel free to add a sheet of lined paper. You may either turn it into the CPP Office Turn In Tray located outside of the office; or email it to .
EMOTIONAL SUPPORT/SERVICE ANIMAL
If you have an Emotional Support or Service Animal and have not gotten approval from the Board; please have your Doctor complete this Confidential Certification Letter.
You will also need to provide a Letter of Request from you stating your name, unit number, a description of the animal requested (cat, dog, and breed); to be submitted along with the Doctor's Confidential Certification Letter.
You may email both letters to . The letters will be forwarded to the General Manager; who will then, submit it for the next Board Meeting*.
*Please note that although, the letters are submitted; there is no guarantee that the Board will get to it on their agenda.
